Registration Documents

Thank you for your interest in registering with Kidz World Preschool, LLC for your child(ren) for the 2025-2026 school year.  

Please read through the registration documents listed below, once you have viewed the documents please go to acknowledgement portion of the registration form to complete the registration by typing in your name. This will take the place of any hand written signatures for all forms except any form that a physician signature is required.

If your child has a food allergy, you will need to download the "Food Allergy Emergency Plan", print and take to your professional physician to complete, then to return to Kidz World Preschool, LLC.

Registration will not be complete until all registration fees have been paid.

Medical Forms

The below forms MUST be signed by a physician in order to be accepted by Kidz World Preschool, LLC. Please download, print, have a physician sign and return to Kidz World Preschool, LLC. 

Current immunization records must be submitted within 2 weeks of registering your child(ren) and on site before they can attend Kidz World Preschool, LLC.
